The Gap + the non-complaint
This happens all the time…
A patient comes in for low back pain. You take the intake. You talk about her sleep, her stress, her bowels. You needle her, she rests, she gets dressed. As she is putting on her shoes she says, "Oh, by the way, I leak a little when I sneeze. But that's just from having kids. It's fine."
It's fine. It's fine.
No, It is not fine.
An acupuncturist who attended one of my classes, calls this the non complaint. The thing patients do not come in for, but mention occassionally. Quietly. Apologetically. With a small laugh. One foot out the door.
She named it perfectly because it names a pattern every one of us sees. The off hand mention. The "by the way." The pelvic floor symptom that has lived in someone's body for ten or fifteen or twenty years without ever being put on an intake form, because nobody ever asked, because the patient herself does not believe it counts.
Here is what I want every acupuncturist reading this to understand. The non complaint is not a small thing. It is the actual reason your patient is in your treatment room.
The leak when she sneezes is connected to the low back pain she came in for. The dyspareunia she has not mentioned to her partner in three years is connected to the anxiety she takes a supplement for. The constipation she calls "just how my body is" is connected to the chronic hip tension that brings her in every six weeks. The pelvic bowl holds everything. The bowl is everything. And our medicine has known this for over two thousand years.
So what do we do.
We ask. Plainly. On the intake form. In the room. Without flinching, without lowering our voice, without making it weird. Bowel habits, bladder habits, sexual function, pelvic pain, pressure, leaking, prolapse symptoms, menstrual pain, postpartum changes. These are not extras. They are the data.
We listen for the by the way. When a patient says "oh, this is probably nothing, but" — that is exactly the moment to put your hand on the doorknob and sit back down. That is the visit you have been waiting for. That is the trust she just handed you.
We treat what she actually came in for, and we name what she did not. Gently. Without shame. With the kind of clinical authority that comes from knowing this is our scope, our medicine, our lineage.
